Authority to provide Medical Information

The PSA has become aware that some members who have submitted ReturnToWorkSA (Workcover) claims are being requested by their injury management team to complete a departmental 'Authority to Exchange Medical Information' form.

The PSA advises members not to complete these departmental forms without first seeking advice from the PSA.

  • The ReturnToWorkSA claim form itself includes a Medical Authority and Worker Declaration. This limits departments to only obtaining information relevant a worker's injury, condition or illness. There is not normally any requirement for a further departmental form. 
  • The PSA is providing this advice to assist members protect their medical information.
